The game of rugby is unique in the fact that there is no official code of rugby or rulebook. It is a collision sport where, as in all sports, head injuries are a common injury. However, the injuries sustained by rugby players are different to those found in other collision sports such as American football, American baseball and American hockey. Within rugby, there are several codes of rugby, one being Test rugby. The biggest difference is that within Test rugby there are international rugby events with the international code of rugby having nearly 60,000 players compared to the international code of rugby having only about 2400 players. International Rugby union is the only code of rugby that devotes a considerable amount of time to injury prevention and has adopted strict rules around what types of headgear are allowed to be worn during an event. Head protection is mandatory during international rugby union.
There are various major rules in rugby that might be considered controversial or even impractical given the current situation. First, since 2010 where the number of men and women eligible to play in a season is equalized, the original 10-team playoff format has been changed to four conference winners with four play-in games to determine the two field teams. Another change occurred in 2011 when the committee responsible for ranking the teams in both the final four and the knockout rounds was abolished and in 2013, to balance the number of games played by the teams, the number of games played per team was reduced from 12 to 10. These changes were designed to help offset the decrease in competitive games. 11
The incidence of a concussion is a substantial concern in the game of rugby. The occurrence of a concussion in rugby is subject to risk factors such as playing surface, speed of play, tackling practices, player position, and experience [ 28 ]. The incidence of concussion following a play is highest for centres and fullbacks due to their position and the frequency of contact [ 17, 29 ], with the lowest rates for fly-half and number 8 who make less contact [ 17, 30 ].
Tackling is the primary source of injury in rugby and contact with the head and neck area is high amongst the most dangerous plays in the game [ 23, 31 ]. Rugby Union is also the leading noncontact sport for concussive injuries [ 32 34 ]. Studies have shown that the incidence of concussion is highest in those players who are involved in tackle play [ 35 ] and that concussions are over three times as common in one hour of football compared to 15 minutes of rugby [ 36 ]. Head injury is an independent risk factor for multiple concussions, and is correlated with drop in performance and increased risk of further injury [ 37 ]. A study published by the New Zealand Rugby Union in 2006 found that 93% of head injuries occurred during playing and tackling [ 38 ].
On the first day of the 2015 season, World Rugby and Rugby Australia implemented a set of changes to the Laws of the Game. These changes have been approved by World Rugby’s council at its headquarters in Dublin, Ireland. The most significant change to the Laws of the Game for 2015 was the introduction of a new phase of play, the ruck. The new laws now allow players to be tackled in the act of falling forward into the ruck. Previously, the two players closest to the ball were the only ones allowed to be tackled. In order to better protect the body, the force of impact has been made less severe. This change is significant not only for ruck play, but for any situation where the head makes contact with the ground. Further, according to World Rugby, this is the first time a head impact rule has been in place across all fifteen regional codes.
